Your CDL Medical Card

Dr. Lisa Barker examines commercial drivers from any state. For a CDL Class A, B, or C or CDL permit you must hold a US DOT Medical Examiner’s Certificate. Forms are available at her convenient office locations in Danbury and New Milford, Connecticut, or can be found on the FMCSA website.

Be Prepared

If you wear glasses or hearing aids or take prescription medication, bring them to your appointment. This will help to expedite the issuance of your CDL Medical Card.
